Being dark-skinned or having oily skin does not always mean having thick skin. When we say \u201cthick skin,\u201d we mean not only the surface of the skin but the density of all layers beneath it. In these patients, the dermis layer is thicker, the subcutaneous connective tissue is denser, and most importantly, the sebaceous glands are highly active.<\/p>\n<p>To understand whether a nose is thick-skinned, we look at some key clues. Especially when we hold the nasal tip with our fingers (pinch test), we feel that the skin does not thin out and comes to the hand like a full roll.<\/p>\n<p>The prominent features of a thick-skinned nose structure are:<\/p>\n<ul>\n<li>Enlarged pores<\/li>\n<li>Orange-peel appearance<\/li>\n<li>Active oiliness<\/li>\n<li>High density of blackheads<\/li>\n<li>Coarse nasal tip<\/li>\n<li>Deep alar grooves<\/li>\n<li>Inelastic skin structure<\/li>\n<\/ul>\n<p>These features directly change our surgical planning. Because this skin structure struggles to adhere to the underlying cartilage framework like a vacuum (shrink-wrap effect). While a thin-skinned nose reveals its shape quickly after surgery, in a thick-skinned nose it takes much longer for the skin to settle onto the framework.<\/p>\n<h2>What should be considered during the preoperative preparation process?<\/h2>\n<p>If your skin is thick, we may need to start the process months before you lie down on the operating table. Because no matter how perfect the surgery is, skin quality is the showcase of the result. Especially if you have oily and acne-prone skin, going under the knife without bringing this under control can be risky.<\/p>\n<p>At this point, dermatologic support comes into play. Certain medical treatments derived from vitamin A\u2014commonly known as acne medications\u2014are of great value for us. These medications reduce sebaceous glands, allowing the skin to thin slightly and become more manageable.<\/p>\n<p>The methods used in preoperative skin preparation are:<\/p>\n<ul>\n<li>Isotretinoin therapy<\/li>\n<li>Professional skincare<\/li>\n<li>Hydrafacial treatments<\/li>\n<li>Glycolic acid products<\/li>\n<li>Retinol-containing creams<\/li>\n<\/ul>\n<p>An important note should be added here: if you are using or will use an isotretinoin-group medication, surgical timing is critical. In the past, it was said that you had to wait 1 year after finishing these medications. However, current medical approaches show that surgery can be performed safely 6 months after stopping the medication. This period is sufficient for healthy wound healing.<\/p>\n<h2>Why is \u201cstructural support\u201d preferred over \u201creduction\u201d in thick-skinned noses?<\/h2>\n<p>This is the most crucial point of thick-skinned rhinoplasty and the part that surprises patients the most. Patients often arrive with the logic, \u201cMy nose is already big, let\u2019s reduce the bones and cartilages a lot so my nose gets smaller.\u201d But in a thick-skinned nose, this mindset can lead to an aesthetic disaster.<\/p>\n<p>Let\u2019s explain the situation with a camping tent example. Imagine you have a very heavy, thick tarpaulin (thick skin). You want to set up a tent that stands upright and neatly with this tarpaulin. If you shorten and weaken the tent poles (the cartilage framework), what happens? That heavy tarpaulin collapses onto the weak poles. The shape of the tent is lost, the top flattens, and an amorphous, broad appearance emerges.<\/p>\n<p>That is why, in thick-skinned noses, we apply the technique called \u201cStructural Rhinoplasty.\u201d Our goal is not to reduce the framework, but on the contrary, to build a strong, solid, and upright framework that can carry that heavy skin. Instead of decreasing the cartilages inside the nose, we support them and reposition them to keep the skin taut from the inside out. Only then can the nose reveal its contours and look refined.<\/p>\n<h2>Why is open technique rhinoplasty more advantageous for thick skin?<\/h2>\n<p>The debate between closed and open techniques in rhinoplasty is always popular, but when it comes to thick skin, the needle generally points toward the open technique. In the open technique, we fully lift the nasal skin through a millimetric incision made in the columella area between the nostrils. This gives us the chance to have full control over all nasal anatomy.<\/p>\n<p>Shaping a thick-skinned nose requires fine craftsmanship and serious \u201cengineering.\u201d We need a wide field of view to place millimetric sutures, correct asymmetries, and most importantly, to securely fix the support grafts we place (cartilage patches). In the closed technique, these maneuvers must be performed in a more limited space.<\/p>\n<p>The advantages of the open technique are:<\/p>\n<ul>\n<li>Full anatomical control<\/li>\n<li>Symmetry control<\/li>\n<li>Ease of graft fixation<\/li>\n<li>Precision in tip shaping<\/li>\n<li>Bleeding control<\/li>\n<\/ul>\n<p>Patients usually worry about a scar on the nasal tip. However, this incision is closed with such a thin suture and with such care that, once healing is complete, it is almost impossible to notice with the naked eye. The aim is to thin the skin slightly so it can settle more easily onto the underlying cartilage framework. This is especially useful in the nasal tip and the supratip area just behind the end of the nasal dorsum.<\/p>\n<p>However, this procedure is a \u201crazor\u2019s edge\u201d balance for the surgeon. Because the capillary network that supplies the skin lies in this layer right under the skin. If one acts too aggressively in an effort to thin the skin and damages this vascular network, the skin\u2019s blood supply can be compromised. This can lead to color changes in the skin, healing problems, and in the worst-case scenario, tissue loss (necrosis).<\/p>\n<p>Therefore, defatting is not applied to every area and is performed strictly within safe limits. The surgeon\u2019s experience and respect for tissue are vital here. Our priority is always patient safety and skin health; aesthetic thinning is pursued only after this safety is ensured.<\/p>\n<h2>What role do cartilage grafts (patches) play in this surgery?<\/h2>\n<p>To create the solid poles in the \u201ctent\u201d analogy mentioned above, we make use of the patient\u2019s own tissues. Septal cartilage obtained from inside the nose is often our first source for strengthening the framework. However, in thick-skinned patients, this cartilage may sometimes be insufficient or weak. In such cases, the need may arise to harvest cartilage from the ear or, very rarely, from the rib.<\/p>\n<p>By placing these cartilage pieces (grafts) at strategic points of the nose, we build a kind of \u201cscaffolding.\u201d This scaffolding makes the nose resistant against contraction forces and gravity over time.<\/p>\n<p>The main graft types used are:<\/p>\n<ul>\n<li>Septal Extension Graft<\/li>\n<li>Columellar Strut Graft<\/li>\n<li>Spreader Graft<\/li>\n<li>Rim Graft<\/li>\n<li>Shield Graft<\/li>\n<li>Cap Graft<\/li>\n<li>Batten Graft<\/li>\n<\/ul>\n<p>Each graft has a different role. Some prevent the tip from drooping, some prevent the nasal wings from collapsing, and some ensure the nasal dorsum looks smooth. All of these pieces come together like parts of a puzzle to form a strong and aesthetic whole.<\/p>\n<h2>Is it possible to make the nasal tip more defined and thinner?<\/h2>\n<p>The biggest dream of thick-skinned patients is to have a thin and refined nasal tip\u2014often described as \u201chazelnut-like.\u201d Within biological limits, it is certainly possible to refine and define the nasal tip, but the techniques used here differ from those for thin-skinned patients.<\/p>\n<p>To achieve a pointed look under thick skin, we need to design the underlying cartilage structure slightly more prominent and sharper than usual. We call this the principle of \u201cover-correction.\u201d On the operating table, the nasal tip is made a bit sharper than the targeted appearance, because we know that the thick skin will soften and mask that sharpness over time as it drapes over it.<\/p>\n<p>Shaping the nasal tip requires not only placing cartilage but also using special suture techniques. We narrow the tip by bringing the cartilage domes closer together and suspending them upward. It is also crucial that there is no dead space between the skin and cartilage. If a space remains, the body fills it with scar tissue and the tip becomes round like a ball. To prevent this, we use a special technique called the \u201csupratip suture\u201d to attach the skin to the cartilage.<\/p>\n<h2>How are wide nasal wings and the nasal base corrected?<\/h2>\n<p>Another common issue in thick-skinned noses is having fleshy and wide nasal wings. When we lift and shape the nasal tip, this width can sometimes become even more noticeable. To achieve an aesthetic balance, it may be necessary to intervene at the nasal base.<\/p>\n<p>To reduce the nasal wings, we perform a procedure called \u201calar base resection.\u201d In this procedure, wedge-shaped tissue is removed either from the fold where the nasal wing meets the cheek or from inside the nostril. This reduces both the size of the nostrils and the width of the nasal base.<\/p>\n<p>The biggest concern here is, again, scarring. However, when we hide the incision line in the natural fold of the nasal wing (the crease line), the scar becomes faint and indistinct after healing. When done with the right technique, this procedure can dramatically change how the nose sits on the face and provide a more refined appearance.<\/p>\n\t\t<\/div>\n\t\t\t\t\n<style>\n#col-703766251 > with thick-skinned rhinoplasty must have is \u201cpatience.\u201d This process is not a 100-meter sprint; it is a marathon. While a thin-skinned patient may begin to see results within 6 months, in a thick-skinned patient it can take 1.5 years, and sometimes even 2 years, for the nose to fully settle into its final form.<\/p>\n<p>The reason is that the lymphatic drainage system works slowly. The incisions made during surgery temporarily affect lymphatic channels. Thick skin and the dense tissue beneath it behave like a sponge that is highly prone to holding fluid. Therefore, postoperative edema (swelling) is much greater and takes much longer to subside.<\/p>\n<p>Possible situations during the recovery process include:<\/p>\n<ul>\n<li>Facial swelling in the mornings<\/li>\n<li>Numbness at the nasal tip<\/li>\n<li>Asymmetric swelling<\/li>\n<li>A feeling of firmness<\/li>\n<li>The nasal tip looking large<\/li>\n<li>Fluctuations in skin tone<\/li>\n<\/ul>\n<p>Swelling generally moves from top to bottom under the effect of gravity. The area between the eyebrows and the nasal root heal first, and the nasal tip is the last to refine. That is why we always tell our patients: \u201cDon\u2019t stare at your nose in the mirror every day; the change will happen not weekly, but in monthly periods.\u201d<\/p>\n<h2>What can be done at home for stubborn edema and swelling?<\/h2>\n<p>After the surgery, a large part of the work falls to the patient. Home-care protocols are very important to protect surgical success and speed up healing. Especially in the first months, fighting swelling should become a lifestyle.<\/p>\n<p>First of all, \u201ctaping\u201d is very valuable for us. After the splint is removed, we teach patients how to tape their nose at night at home. These tapes apply pressure to the skin, help disperse swelling, and encourage the skin to adhere to the cartilage.<\/p>\n<p>Foods that help reduce swelling are:<\/p>\n<ul>\n<li>Pineapple<\/li>\n<li>Parsley<\/li>\n<li>Cherry-stem tea<\/li>\n<li>Green tea<\/li>\n<li>Cucumber<\/li>\n<li>Plenty of water<\/li>\n<\/ul>\n<p>In addition to nutrition, gentle lymphatic massages performed after the 1st month as instructed by your doctor help drain accumulated fluid. Also, sleeping with a high pillow reduces morning swelling by keeping the head above heart level.<\/p>\n<p>Sometimes, despite all this, stubborn firmness and fullness can remain behind the nasal tip (the supratip area). In that case, after the 3rd or 4th month, we may perform a very low-dose cortisone injection (steroid injection) into that area. This injection helps dissolve the scar tissue accumulated there and makes the nose look more refined.<\/p>\n<h2>What are the complication risks, and is revision necessary?<\/h2>\n<p>As with any surgical procedure, thick-skinned rhinoplasty also carries some risks. The most common issue we encounter in thick-skinned patients is a condition we call \u201cSupratip Polly Deformity.\u201d This is when the area just behind the nasal tip remains swollen, and when viewed from the side, the nose can look like a parrot beak. This may be caused by insufficient cartilage support or by scar tissue accumulating in a dead space.<\/p>\n<p>Other possible risks are:<\/p>\n<ul>\n<li>Nasal tip drooping<\/li>\n<li>Persistent edema<\/li>\n<li>Skin discoloration<\/li>\n<li>Asymmetry<\/li>\n<li>Inability to achieve the desired thinness<\/li>\n<\/ul>\n<p>If a corrective (revision) surgery is needed, it should not be rushed. In thick-skinned patients, tissues should be allowed to heal and soften completely. This period is at least 1.5\u20132 years. An early intervention traumatizes the healing tissues further and can worsen the outcome.<\/p>\n<h2>How do lifestyle habits affect the result?<\/h2>\n<p>You had the surgery and everything went well\u2014but what about after? Your daily habits directly affect the final appearance of your nose. Especially smoking is the biggest enemy of this surgery. Smoking reduces oxygen in the blood and causes the smallest vessels at the extremities to constrict. Thick skin, which is already difficult to nourish, cannot receive enough oxygen when you smoke, and healing is disrupted.<\/p>\n<p>Other lifestyle factors to pay attention to are:<\/p>\n<ul>\n<li>Protection from the sun<\/li>\n<li>Restricting salt intake<\/li>\n<li>Avoiding heavy exercise<\/li>\n<li>Staying away from hot environments<\/li>\n<li>Wearing glasses<\/li>\n<\/ul>\n<p>Especially in the first 6 months, using sunscreen and wearing a hat are very important.
