Whatever the main pole in the middle is that keeps this tent standing, that is what the septum is for your nose. From the outside, this structure, located right in the center of our face, divides the nasal cavity into two separate tunnels, right and left. However, this division is not a random separation; it is an aerodynamic arrangement.<\/p>\n<p>Anatomically, the septum is not made up of a single solid block. In the front part, there is the quadrangular cartilage that provides the flexibility you feel when you touch the tip of your nose. As we move further back and upward, this structure becomes firmer and turns into bone. In the posterior upper part, a portion of the ethmoid bone comes into play, and in the lower part, the bony structure called the vomer is involved. These three different components unite along a line at the base of the nose in perfect harmony, like pieces of a puzzle.<\/p>\n<p>The septum\u2019s role is not only to divide the nose into two. Its main vital function is to ensure the \u201claminar\u201d flow of the air we breathe. Laminar flow means that air moves smoothly, regularly, and without resistance, like a river. With a properly aligned septum, air enters through the nostrils, warms, humidifies, and gets filtered as it moves backward. It does not encounter any obstacles. But when there is a deviation in the septum, this smooth river suddenly turns into a turbulent waterfall. The flow is disrupted, turbulence begins, and breathing quality drops dramatically.<\/p>\n<h2>Why Does Septal Deviation Occur and How Does It Affect the Body?<\/h2>\n<p>The condition we medically call \u201cseptal deviation\u201d is when this central wall shifts to the right or left, bends, or forms sharp protrusions (spurs) at certain points. So why do some of us have an internally crooked nose? The answer generally falls into two main categories: our genetic inheritance and accidents in everyday life.<\/p>\n<p>In some patients, this condition is completely congenital. Even the position in the womb or pressures during passage through the birth canal can determine the septum\u2019s fate. If our genetic code predisposes us to facial asymmetry, as we age and nasal bones grow, the septum may fail to keep up with that pace and bend. However, the most common cause we see is childhood falls and bumps. A minor trauma that was not taken seriously at that age can affect the growth centers (microscopic factories) of the nose. As the child grows, the nasal framework develops, but the septum grows asymmetrically due to the damage it sustained.<\/p>\n<p>When a deviation occurs, chaos begins in the body. According to the laws of physics, on the narrower side where the deviation is, air becomes compressed and its speed increases. Faster-moving air dries out the area it passes through. This leads to crusting, burning, and frequently recurring nosebleeds.<\/p>\n<p>But the most interesting part is what we call \u201cparadoxical congestion.\u201d The patient usually expects the deviated side to be blocked. Yet congestion is also experienced on the non-deviated, wider side. Why? Because our body does not like empty space. To fill the larger cavity, the turbinates (nasal conchae) swell and enlarge. As a result, the patient is left with a nose that has lost function: one side is closed due to the bony deviation, the other due to turbinate enlargement.<\/p>\n<p>The main problems caused by septal deviation are:<\/p>\n<ul>\n<li>Chronic nasal congestion<\/li>\n<li>Sleeping with the mouth open<\/li>\n<li>Waking up tired in the mornings<\/li>\n<li>Frequently recurring throat infections<\/li>\n<li>Decreased exercise capacity<\/li>\n<li>Impaired sense of smell<\/li>\n<li>Postnasal drip<\/li>\n<li>Recurring nosebleeds<\/li>\n<li>Facial and headache pain<\/li>\n<\/ul>\n<h2>How Is the Nasal Cleaning Mechanism Damaged?<\/h2>\n<p>Our nose is not just an air tunnel; it is also a highly advanced filtration system. The mucosal layer lining the inside of the nose is covered with millions of microscopic hairs (cilia) that are invisible to the naked eye. These hairs continuously sweep dust, germs, and particles that stick to them from the front of the nose toward the back, to the nasopharynx. We call this system \u201cmucociliary clearance.\u201d It is the nose\u2019s self-washing and self-cleaning mechanism.<\/p>\n<p>In a healthy person, this sweeping process is very fast and effective. A dust particle entering from outside is carried to the back of the nose within approximately 10\u201315 minutes and is swallowed and destroyed by stomach acid. However, when there is a septal deviation, airflow is disrupted and turbulence occurs. This turbulence crushes and sheds those delicate cilia in that area, like a strong wind flattening crops. In addition, the dryness caused by airflow disrupts the consistency of mucus, making it sticky rather than fluid.<\/p>\n<p>Scientific measurements show us this: in patients with severe deviation, the nose\u2019s self-cleaning time can reach 25\u201330 minutes, and sometimes this system stops completely. What happens then? Bacteria and viruses remain in the nose longer and have time to multiply. This explains why the patient is constantly sick and why sinusitis attacks simply do not resolve. Just because the septum appears deviated on radiological images does not mean the person must undergo surgery. If the person has no complaints, sleeps well, and has good exercise capacity, we do not touch that deviation.<\/p>\n<p>However, there are situations where surgery is no longer a choice but a necessity for health. Our most basic criterion is deterioration in the patient\u2019s quality of life. A nose that does not open despite medical treatments, nasal sprays, and allergy therapies indicates a mechanical obstruction, and mechanical problems can only be solved with mechanical (surgical) methods.<\/p>\n<p>The main conditions that confirm the decision for surgery are:<\/p>\n<ul>\n<li>Nasal congestion resistant to medical treatment<\/li>\n<li>Chronic sinusitis attacks<\/li>\n<li>Severe snoring<\/li>\n<li>Obstructive sleep apnea syndrome<\/li>\n<li>Uncontrollable nosebleeds<\/li>\n<li>Persistent one-sided headaches<\/li>\n<li>Middle ear problems<\/li>\n<li>Obvious external deviation<\/li>\n<\/ul>\n<p>In addition, sometimes even if the main issue is not nasal congestion, we may need to perform septoplasty to treat another condition. For example, if a pituitary tumor will be accessed through the nose or if a tear duct obstruction surgery will be performed, the septum may need to be corrected to open the surgeon\u2019s working space. In such cases, septal surgery serves as a step on the path to treatment.<\/p>\n<h2>What Is the Ideal Age for Surgery and How Should Children Be Approached?<\/h2>\n<p>The concept of the \u201cright time\u201d is quite critical for this surgery. The generally accepted rule is to wait until around age 18, when facial development is largely complete. Why don\u2019t we rush? Because in certain regions of the septum there are special growth centers that allow the nose and midface to develop. Surgery performed at an early age, especially with an aggressive technique, can damage these centers. As a result, the child\u2019s nose may not grow, the face may remain flattened, and a collapsed nasal structure called \u201csaddle nose\u201d can occur.<\/p>\n<p>However, in medicine we do not like to use the word \u201cnever.\u201d In some exceptional cases, waiting until 18 can cause greater harm to the child. If the deviation completely prevents the child from breathing, causes severe sleep apnea, leads to growth and developmental delay, or if the dental and jaw structure has begun to deteriorate due to mouth breathing, we cannot afford to wait.<\/p>\n<p>In these special cases, we use a very delicate method called \u201cProtective Pediatric Septoplasty\u201d (Limited Septoplasty). In this surgery, the surgeon proceeds as carefully as if walking through a minefield. The growth centers are not touched and large cartilage removals are not performed. Only the most obvious deviation that blocks the airway is corrected with minimal interventions. The goal is to enable the child to breathe comfortably until the end of adolescence. If necessary, a more detailed revision surgery can be planned after age 18.<\/p>\n<h2>What Are Modern Surgical Techniques and Methods?<\/h2>\n<p>Over the years, medical technology and our surgical philosophy have changed greatly. In the past, septoplasty often meant removing and discarding a large portion of the deviated cartilage and bone. This approach did open the airway, but it weakened the structural support of the nose. Over time, tip drooping and dorsal collapse could occur. Today, our approach is built not on \u201ctear down and discard,\u201d but on \u201crepair and reshape.\u201d Our goal is no longer to throw cartilage away, but to correct it and put it back in place\u2014because that cartilage is the most valuable building block of your nose.<\/p>\n<p>We can list the main techniques we use today as follows:<\/p>\n<ul>\n<li>Closed Technique and the Cottle Method:<\/li>\n<\/ul>\n<p>This method is the foundation of classic septoplasty. Through a hidden incision inside the nostril, the mucosa is gently separated from the cartilage. The deviated cartilage is not removed; instead, it is straightened by crushing with special instruments, tension is released by making scoring incisions, or it is suspended to the midline with sutures. In other words, the cartilage is disciplined and left in the position where it should be.<\/p>\n<ul>\n<li>Endoscopic Septoplasty:<\/li>\n<\/ul>\n<p>This is one of the greatest gifts technology has given us. We enter the nose using thin rods (endoscopes) with a camera and light at the tip. This allows us to see even the most posterior, darkest deviations in HD quality on a large screen. The biggest advantage is that it enables us to intervene only in the problematic area. Instead of peeling the entire nose, we perform targeted surgery. This results in less postoperative swelling and a very fast recovery. It is the gold standard especially for removing bony protrusions called \u201cseptal spurs.\u201d<\/p>\n<ul>\n<li>Extracorporeal Septoplasty:<\/li>\n<\/ul>\n<p>This method represents the peak of surgery. It is usually applied in patients with severe trauma, an \u201cS\u201d or \u201cZ\u201d shaped deformity, or a crooked nose. When working inside the nose is insufficient, we remove the septal cartilage as a whole. On the sterile back-table, we reconstruct the cartilage like a sculptor or engineer. We cut the curvatures, patch it, support it with resorbable plates, and after turning it into a perfectly straight plate, we re-implant it into the nose. This allows us to create miracles even in noses that seem hopeless.<\/p>\n<h2>Why Is the Septum Valuable for Facial Aesthetics and Rhinoplasty?<\/h2>\n<p>As an ENT and Facial Aesthetic Surgeon, my view of the septum goes far beyond it being just a wall. For us, the septum is an invaluable \u201cspare parts bank.\u201d In aesthetic nose surgery (rhinoplasty), the most ideal material to lift the tip, straighten the dorsum, and strengthen the nasal wings is the patient\u2019s own septal cartilage. It is straight, firm but moldable, and carries zero risk of rejection by the body.<\/p>\n<p>If a careless septoplasty was previously performed and the cartilage was unnecessarily discarded, things become more difficult for a patient who later wants aesthetic surgery\u2014because we no longer have material to use. In that case, we have to harvest graft material from the ear or rib cartilage. the scan shows the inside is open, but I still can\u2019t breathe.\u201d At this point, the culprit is often the \u201cnasal valve\u201d area. The nasal valve is the narrow angle between the cartilages that form the nasal roof and the septum. It works like a clamp. Normally, this angle should be 10\u201315 degrees during inhalation. If it has narrowed, no matter how deeply you try to breathe, due to the vacuum effect the nasal wings collapse inward and airflow is cut off.<\/p>\n<p>To solve this problem, we use cartilage obtained from the septum very intelligently. We place cartilage strips called \u201cSpreader Grafts\u201d between the septum and the roof cartilages. Just like stretching a tight shoe with a shoe tree, this widens that narrow angle. Thus, the breathing problem is solved and the delicate aesthetic lines on the nasal dorsum (dorsal aesthetic lines) become more defined.<\/p>\n<p>In addition, nasal tip drooping also makes breathing more difficult. If you notice that you breathe better when you gently lift the tip of your nose with your finger, it means you need tip support. We provide this with a \u201cColumellar Strut\u201d graft prepared from septal cartilage. This graft acts like a pillar placed between the two nostrils, preventing the nasal tip from dropping over time.<\/p>\n<p>The main grafts we use in valve surgery are:<\/p>\n<ul>\n<li>Spreader grafts<\/li>\n<li>Columellar strut graft<\/li>\n<li>Septal extension graft<\/li>\n<li>Alar batten grafts<\/li>\n<li>Rim grafts<\/li>\n<\/ul>\n<h2>How Is Nasal Packing Used After Surgery?<\/h2>\n<p>When septoplasty is mentioned, everyone thinks of that nightmare: \u201cWhen they pulled out the packs, I felt like my brain moved!\u201d Fortunately, this sentence has largely remained in the dusty shelves of medical history. The long gauze packs used in the past were truly painful to remove because they dried and stuck to tissue. However, in modern medicine, patient comfort comes first.<\/p>\n<p>Today we use two main methods, both quite comfortable:<\/p>\n<p>Grooved Silicone Splints:<\/p>\n<p>These are leaf-shaped, soft and slippery silicone materials. Their biggest feature is that they have air tunnels in the middle. So the patient can breathe through these tunnels as soon as they wake up from surgery. They do not adhere to tissue. Removal takes seconds, and the patient feels not pain but rather a strange tickling sensation as something slides out of the nose.<\/p>\n<p>Pack-Free Surgery (Transseptal Suture):<\/p>\n<p>In suitable cases, we may not use any packing or silicone at all. With a special technique called the \u201cquilting suture,\u201d we stitch the two mucosal layers of the septum along their length, like sewing a quilt. These stitches prevent blood from collecting in between and adhere the tissues together. The patient leaves surgery without any foreign material in the nose. This technique is the peak of patient comfort, but not every nasal anatomy is suitable; the surgeon decides this during the operation.<\/p>\n<p>Things to pay attention to after surgery are:<\/p>\n<ul>\n<li>Regular nasal rinsing with prescribed sprays<\/li>\n<li>Keeping the inside of the nose moist<\/li>\n<li>Avoiding heavy sports<\/li>\n<li>Staying away from hot baths and steam<\/li>\n<li>Protecting the nose from impacts<\/li>\n<li>Avoiding forceful blowing<\/li>\n<\/ul>\n<h2>What Are Possible Complications and Risks?<\/h2>\n<p>Septoplasty is a safe procedure with an extremely low complication rate when performed by experienced hands. However, as with any surgical intervention, it is not possible to talk about zero risk. It is important for the patient to know these possibilities for transparency.<\/p>\n<p>The condition we worry about most is a \u201cseptal hematoma.\u201d This is a collection of blood between the mucosal layers. Septal cartilage is an avascular tissue; it receives its nutrition through diffusion from the mucosa covering it. If blood accumulates in between, the cartilage cannot be nourished and may dissolve within 48\u201372 hours. This can lead to permanent nasal deformity. Therefore, if there is sudden severe pain and complete blockage in the nose after surgery, you must urgently contact your doctor. The problem is resolved with a simple drainage procedure.<\/p>\n<p>Another risk is \u201cseptal perforation,\u201d meaning the formation of a hole in the septum. If opposing mucosal tears occur during surgery and go unnoticed, a hole may remain as it heals. This can cause a whistling sound when breathing and constant crusting. An experienced surgeon will notice and repair a tear even if it occurs during the procedure.<\/p>\n<p>Another rare complication is \u201csaddle nose\u201d deformity. This is the collapse of the nasal bridge due to excessive weakening of the L-strut support. This turns a functional surgery into an aesthetic problem, and correcting it usually requires a more extensive revision surgery using rib cartilage. What should you expect? In the first few days, mild swelling in the face and a feeling of nasal congestion are very normal. Pain is usually minimal and can be controlled with simple painkillers. Our patients often describe it as \u201cit feels like I\u2019m having a bad cold.\u201d<\/p>\n<p>Silicone splints are usually removed on the 2nd or 3rd day after surgery, and sometimes depending on the situation on the 7th day. The moment the splints are removed is a milestone for our patients. The smile that spreads across their faces when they take their first deep breath is the biggest reward for us as physicians. We often hear the sentence, \u201cDoctor, I felt oxygen reach my brain.\u201d<\/p>\n<p>However, healing does not end here. It can take 3 to 6 weeks for the nasal mucosa to fully renew itself and for the cilia to start working again. During this period, nasal irrigations, saline solutions, and moisturizing drops will be your closest friends. Cleaning crusts speeds up healing.<\/p>\n<p>How do we measure the outcome of a successful surgery? Simply saying \u201cmy nose opened up\u201d is a subjective comment. Scientifically, we score the increase in the patient\u2019s quality of life using questionnaires such as NOSE and SNOT-22.
