Sometimes the problem is very obvious; there is an irregularity on the nasal bridge that can be felt by hand or a clear deviation when viewed from the front. Sometimes the problem is not very noticeable from the outside, but the patient cannot breathe, sleeps with their mouth open at night, and has poor quality of life. Although the signs that things did not go well after the first surgery vary from person to person, the main complaints are generally a combination of functional difficulties along with aesthetic concerns.<\/p>\n<p>The most common reasons patients seek revision are as follows:<\/p>\n<ul>\n<li>Nasal tip drooping<\/li>\n<li>Collapse of the nasal bridge<\/li>\n<li>Persistent nasal deviation<\/li>\n<li>Difficulty breathing<\/li>\n<li>Nostril asymmetry<\/li>\n<li>Over-rotated nasal tip<\/li>\n<li>Collapse of the nasal wings<\/li>\n<li>Parrot beak deformity<\/li>\n<li>Pointed and unnatural appearance<\/li>\n<\/ul>\n<p>Each of these items is actually the result of certain setbacks experienced during surgery or the healing process. The condition we call a &#8220;parrot beak&#8221; is the bulging appearance that occurs because the cartilage and tissues just behind the nasal tip are not reduced sufficiently. However, the most critical issue is breathing problems. Thinning the nasal roof too much for the sake of aesthetics or narrowing the internal air valves (valves) can lead to narrowing in the nose, condemning the patient to lifelong nasal obstruction. The goal of revision surgery is to identify these damages and make the nose an organ that both looks pleasing and sends full oxygen to the lungs.<\/p>\n<h2>Why is the revision rhinoplasty process more difficult than the first surgery?<\/h2>\n<p>Sometimes our patients may think, &#8220;Doctor, we&#8217;re only going to do a small touch-up, why is it so complicated?&#8221; However, revision surgery is technically a very different and more challenging process than the first operation. Explaining this with a construction example is always easier to understand. The first surgery (primary rhinoplasty) is like building a brand-new building from scratch on an empty, clean plot, according to its project. The foundation is solid, the materials are complete, and you start everything from zero.<\/p>\n<p>Revision rhinoplasty, on the other hand, is like trying to restore a building whose columns are damaged, walls are cracked, and plan is distorted, without demolishing it. When the surgeon starts the operation and lifts the skin, they may not know what they will encounter inside. In the first surgery, cartilage may have been excessively removed, bones may have healed asymmetrically, or tissues may have become adherent. The surgeon\u2019s task here is not only to shape, but first to clear the field, identify the damage, and then strengthen the structure by completing the missing parts.<\/p>\n<p>The factors that increase this difficulty are as follows:<\/p>\n<ul>\n<li>Skin adhesions<\/li>\n<li>Distorted anatomy<\/li>\n<li>Insufficient remaining cartilage<\/li>\n<li>Impaired blood circulation<\/li>\n<li>Loss of skin elasticity<\/li>\n<\/ul>\n<p>In particular, the scar tissue produced during healing is the surgeon\u2019s greatest challenge. After the first surgery, the body produces a tough, fibrous tissue to repair that area. This tissue sticks anatomical planes together and limits the surgeon\u2019s millimetric working space. In addition, the blood circulation of previously operated skin is more delicate than untouched skin. Performing revision surgery without damaging the skin is quite difficult. That is why revision surgery cannot be rushed; it requires much more meticulous, slow, and careful work. The generally accepted, recommended, and scientifically supported time for revision surgery is at least one year after the first operation.<\/p>\n<p>So, why one year? Because the healing process of the nose does not progress as quickly as it appears from the outside. In the first months, the nose is still swollen. This swelling masks the true shape of the underlying bone and cartilage structure. An assessment made while swelling is present can be misleading; asymmetries or depressions that will appear when the swelling subsides may not yet be visible. Even more importantly, there is the internal wound-healing process. During the first 6\u20139 months, the tissues are still in an &#8220;active&#8221; healing phase. In this period, the tissues are firm, highly vascular, and not suitable for surgical intervention.<\/p>\n<p>The risks of early intervention are as follows:<\/p>\n<ul>\n<li>Excessive bleeding<\/li>\n<li>Impaired skin nutrition<\/li>\n<li>Permanent tissue damage<\/li>\n<li>Unpredictable results<\/li>\n<\/ul>\n<p>If the tissues are reopened before they fully soften (&#8220;mature&#8221;), skin nutrition can be disrupted, which can lead to irreversible skin problems. Also, working with firm tissues makes it impossible for the surgeon to shape effectively. Of course, there may be exceptional urgent situations, such as complete airway obstruction or infection. However, in aesthetically focused corrections, patience is the key to success. At the end of one year, swelling fully subsides, tissues soften, and the nose reaches its final form. That is when planning and surgery provide the healthiest and most lasting result.<\/p>\n<h2>How should the psychological process be managed before revision rhinoplasty?<\/h2>\n<p>Being a revision patient requires a very different psychology compared to primary (first-time) patients. In your first surgery, you were excited and hopeful, but the result was disappointment. This can naturally undermine your trust in physicians and the surgical process. It is completely normal to feel anxious, skeptical, and unhappy. Therefore, the revision process is not only a physical repair but also an emotional repair process.<\/p>\n<p>The most important step in this process is to place your expectations on a realistic ground. With the trauma created by the first surgery, patients sometimes enter a search for a &#8220;perfect&#8221; and &#8220;flawless&#8221; nose. However, in revision surgery, the goal should not be &#8220;perfection&#8221; but reaching &#8220;better&#8221; than the current situation. Because the tissue material we have is limited and has been damaged before. Open communication with your surgeon is vital at this point. You should move forward with a physician who truly listens to you, understands your concerns, and honestly explains what can (and cannot) be done.<\/p>\n<p>The signs of Body Dysmorphic Disorder (Dysmorphic Disorder) are as follows:<\/p>\n<ul>\n<li>Being obsessed with minor flaws<\/li>\n<li>Social isolation<\/li>\n<li>Constantly changing doctors<\/li>\n<li>Unrealistic expectations<\/li>\n<\/ul>\n<p>Making use of technology is also very effective in reducing this anxiety. 3D simulation devices (for example, Vectra) allow you to see the possible outcome of your nose on your own face before surgery. This makes abstract questions like &#8220;Will my nose be too upturned?&#8221; or &#8220;How much will the hump be removed?&#8221; more concrete. However, remember that this is a computer rendering; living tissue does not always match the computer millimetrically. Still, it is the best way to understand whether you and your surgeon speak the same aesthetic language. If you are very unhappy with your reflection in the mirror, going through this process with psychological support can also be very helpful. A surgery entered without being mentally ready may not satisfy you, no matter how good the result is.<\/p>\n<h2>Is it necessary to use rib cartilage in revision rhinoplasty?<\/h2>\n<p>When revision rhinoplasty is mentioned, the issue that most often startles patients is usually the matter of &#8220;rib cartilage.&#8221; We frequently hear the question, &#8220;Doctor, why is a piece taken from my rib for my nose?&#8221; This fear is understandable; at first it sounds like a big and painful procedure. However, the truth is that rib cartilage is the greatest lifesaver and insurance policy of revision surgery.<\/p>\n<p>Think of the nose like a tent. This tent needs poles (cartilage) to stay standing. In your first surgery, the septal cartilage inside the nose, which is your own spare-part reserve, has usually been used or damaged. In other words, we no longer have enough &#8220;construction material&#8221; to rebuild the nose. Ear cartilage is an option, but by nature it is soft and curved. It is excellent for tip shaping, but it is not strong or straight enough to correct the nasal bridge or to raise a collapsed nose.<\/p>\n<p>The advantages of rib cartilage are as follows:<\/p>\n<ul>\n<li>It is available in abundant quantity<\/li>\n<li>Its structure is straight and strong<\/li>\n<li>It can be shaped easily<\/li>\n<li>It is long-lasting<\/li>\n<\/ul>\n<p>Rib cartilage (costal cartilage) is generally taken through a small 2\u20133 cm incision made under the right chest, hidden in the inframammary fold in women. During this procedure, the lungs are never touched and no breathing-related problem occurs. This strong cartilage is then sculpted by the surgeon like an artist. Sometimes it is used in thin plates to support the nose, and sometimes it is cut into tiny cubes with the &#8220;diced cartilage&#8221; method and placed on the nasal bridge within a sheath. With these methods, your nose maintains its form over the years, does not collapse, and does not bend. There may be mild pain in the rib area for a few days after surgery, but this is easily controlled with simple painkillers and does not restrict your daily However, when it comes to revision, the vast majority of surgeons worldwide favor the open technique. So, why?<\/p>\n<p>The answer is simple: visual control. In revision surgery, the internal anatomy is distorted, adhesions have formed, and symmetry has been lost. In order to correct this complex picture, the surgeon must not perform a &#8220;blind flight&#8221; and needs to see everything clearly. The open technique allows the nasal skin to be lifted through a very small incision in the columella area between the nostrils. In this way, the nasal framework is fully exposed.<\/p>\n<p>The advantages provided by the open technique are as follows:<\/p>\n<ul>\n<li>Clear identification of the damage<\/li>\n<li>Fixation of grafts<\/li>\n<li>Correction of asymmetries<\/li>\n<li>Bleeding control<\/li>\n<\/ul>\n<p>Especially if additions such as rib cartilage will be made, these pieces must be secured to the main framework with millimetric sutures.
